Enrollment Information
New Student Enrollment Forms and Calendars
Preschool Requirements:
- Certified Birth Certificate (will not accept hospital copy or wallet-size copy)- within 30 days
- Copy of Social Security Card (optional)
- Up-to-date Kentucky Immunization Certificate including varicella vaccine (chickenpox)
- Vision examination completed by optometrist or ophthalmologist-(required for first time in public school)
- Current Physical Examination (within 6 months prior to enrollment)
- Proof of Residence
- 4 year old: Completed Income Eligibility Form
Kindergarten Requirements:
- Certified Birth Certificate (not a hospital copy or wallet-size copy)-within 30 days
- Copy of Social Security Card
- Up-to-date Kentucky Immunization Certificate including second MMR and Hepatitis (HEB) B (series of 3 doses) and varicella vaccine (chickenpox).
- Vision examination completed by optometrist or ophthalmologist- (required for first time in public school)
- Current Physical Examination (within 1 year prior)
- Proof of Residence
- Dental Screening completed by a dentist, dental hygienist, physician, registered nurse, advanced registered nurse practitioner, or physician assistant (required first year that a 5 or 6-year-old is enrolled in public school)
First Time Enrollees in Kentucky Schools:
- Certified Birth Certificate (not a hospital copy or wallet-size copy)-within 30 days
- Copy of Social Security Card
- Up-to-date Kentucky Immunization Certificate - upon enrollment
- *Current Physical Examination (within 1 year prior)
- Kindergarten: Exam within 1 year prior to enrollment
- 1st - 5th Grade: Proof of initial exam from the previous district or must obtain one & can enroll with an appointment card. Must have within 30 days of enrollment.
- 6th - 12th Grade: Proof of 6th-grade exam or must obtain one & can enroll with an appointment card. Must have within 30 days of enrollment.
Students Entering 6th Grade:
- Up-to-date Kentucky Immunization Certificate- current & to include the second MMR vaccine and the Hepatitis (HEP) B (series of 3 doses)
- Second Physical Examination - within one year prior to entering 6th grade
- Students entering 6th grade must have the meningococcal vaccine
All Students:
MUST HAVE AN UP-TO-DATE (CURRENT) KENTUCKY IMMUNIZATION CERTIFICATE ON FILE.
* Physical Exams not on a Kentucky form must include the following:
- Medical history;
- An assessment of growth, development & general appearance;
- A physical assessment including hearing & vision screening;
- Recommendations to the school regarding health problems that may require special attention in the classroom or physical education activities.
Meade County High School Graduation Requirements:
Grade Classification: Credits required to be promoted to the next grade level.
9th - 10th Grade: Minimum 5-7 credits
10th - 11th Grade: Minimum 11-14 credits
11th - 12th Grade: Minimum 17-21 credits
Graduation: Minimum 24-28 credits
Graduation Credits:
English - 4 credits: English I, II, III, IV
Math - 4 credits: Algebra I & II, Geometry, Math Elective - 1
Science - 3 credits: Earth/Space, Life, Physical
Social Studies - 3 credits: Human Geography, European History, US History
Health/Physical Educ - 1 credit
Career Survey/Digital Literacy - 1 credit
Humanities - 1 credit
Total Minimum Credits - 24 credits
