Eligibility: Enrollment occurs year-round.

• 4-year-olds eligible whose family income is no more than 160% of the poverty

• 3 and 4-year-olds with a disability or developmental delays

Disability Areas:

• Children are screened in the following areas

• Cognitive

• Motor

• Language

• Social/Emotional

• Self-help/Adaptive

Required Documents for Preschool Registration

• Proof of Residence

• Up-to-date Kentucky immunization certificate

• Physical examination certificate signed by a doctor

• Certified Birth Certificate (not the hospital copy)

• Eye Exam (Certified Optometrist)
